This helps us tailor your lesson to your goals and includes our required safety and liability acknowledgment.

With this free form, you'll also receive access to a downloadable PDF and Google Form version of the waiver and lesson details. You can choose to submit an online Google Form, print the PDF and bring it to your first lesson OR email it to us ahead of time as an alternative to the online form.
